Final Evaluation Forms

These evaluation forms can be opened and completed in Microsoft Word. If you do not have Microsoft Word on your computer please download the .pdf files at the bottom of the page, print them, and then complete them.

When you select the link to access the papers - you are starting to download the file to your personal computer. Please choose the ¨save as¨ or ¨save file¨or ¨save it to disk¨ option. Save the file in a folder on your computer. Double click on the file you have just downloaded, and it should automatically open in Microsoft Word or Adobe Reader.

Microsoft Word Evaluation Forms
Fill in your answers in the grey boxes provided.  These grey boxes will not print, they just help you know where to enter information. After you have completed the form:

1. Save the form as the student teachers first and last name (ex. John_Smith). Please do not use any spaces when naming the file.
2. Attach the completed form in an email to the UNI coordinator/supervisor.
