Individuals who have a short term condition that substantially limits their ability to perform one or more major life activities related to the academic environment may be eligible for temporary accommodations through Student Disability Services.
REQUEST PROCEDURE FOR TEMPORARY ACCOMMODATIONS
If you believe you have a short term condition that is substantially limiting, contact Student Disability Services to schedule a consultation appointment. In addition to meeting with a staff member, individuals wishing to request temporary accommodations should complete the Request for Services and Documentation Review Form as fully as possible. Students should also submit documentation from a qualified healthcare provider that supports the request for temporary accommodations. This information can be provided by having the healthcare professional complete the Verification of Temporary Disability Form. Once completed, both pieces of information should be sent to:
