Academic Verification Request

The Academic Verification Request is utilized by student who are looking for any of the following: (1) Address Verification; (2) Enrollment Verification (3) Good Student Discount; or (4) Loan Deferment Request. Students can now access an Enrollment Verification at anytime with Student Self-Service - no more waiting for us to process your request! Academic/Administrative Fee Request Form

When adding a academic/administrative fee, please complete the justification section providing an explanation of what the fee will be used for and a detailed accounting of the expenses to be covered by the fee. This form must be approved via the appropriate channels prior to its being processed by the Office of the Registrar

Ad Hoc Report Request

The Ad Hoc Report Request is utilized by students, faculty, staff, or outside constituents who in need for information that is not already provided elsewhere by the university. Prior to filling out and submitting this WebForm, please take a moment to review the information on our Reports & Statistics webpage and also on the Office of the Institutional Research website.

Application for Readmission from Suspension

The Application for Readmission from Suspension is utilized by a student who wishes to return to the University of Northern Iowa as a student after they have been academically suspended. For more information on Readmission from Suspension, please click here.

Change of Address Form

Students can utilize this form to change their school session, interim, or home/parent address. Once completed, the form should be submitted to our office for processing. Students can also change their address electronically through their MyUNIverse account by taking the following steps: (1) Log into your MyUNIverse account; (2) Click on the "My Page" tab; (3) Under "Update My Personal Information" click the "Update My Personal Information" link; (4) Re-enter your log-in credentials; and (5) Select the appropriate informational tab along the top of the "Personal Information" display.

Class Roster Verification Form

The Class Roster Verification Form is utilized by instructional staff to verify class registration. Please compare your current class roster(s) in MyUNIverse with the students actually attending class. If there are discrepancies between class roster(s) and student attendance, please note them on this form. If there are courses in which students may earn differing amounts of credit, please note this in the "Hours Credit" area. If there are no discrepancies to report, please complete and submit this form to indicate that you have verified your class roster(s).

Classroom Reservation Request

The Classroom Reservation Request is utilized by students, faculty, and staff who would like to request the use of a general classroom for an academic class, such as an additional meeting time, an alternate (or secondary) exam room, a break out room, review sessions, class presentations, or as a space for student organization meetings. Course/Lab/Studio and Miscellaneous Fee Request Form

Please complete each section of this form. When adding a new course/lab/studio or miscellaneous fee or increasing and existing course/lab/studio or miscellaneous fee, please complete the justification section providing an explanation of what the fee will be used for and a detailed accounting of the expenses to be covered by the fee.

Declaration of Curriculum

A Declaration of Curriculum is filed whenever you are declaring, dropping, adding, or changing a major or minor. The major department that offers the major must approve this Declaration and assign you a major advisor. Both you and your major advisor must sign the Declaration of Curriculum. If you are declaring a second major, the second major advisor must also sign. If you wish to continue as a Deciding student, the Office of Academic Advising should sign the Declaration of Curriculum. If you wish to continue as a prospective major, your current advisor should sign the Declaration of Curriculum. Declarations of a minor do not require your advisor's signature. The completed Declaration of Curriculum must be filed with the Office of the Registrar before processing can take place.

Diploma Reorder Form

The Diploma Reorder Form is utilized by an individual who wishes to reorder an official university diploma. For your convenience, a Diploma Reorder Form can also be fill out and submitted electronically. If you would like to submit a Diploma Reorder Form electronically, please click here. Prior to submitting the Diploma Reorder Form (either as a hard copy or electronically), it is strongly encouraged that you read Requesting a Replacement Diploma under the "Alumni & Friends" tab.

Directory Information Change

This form can be used to restrict the University from releasing Directory Information items to outside parties. Unless otherwise noted, all Directory Information items will be made available upon request.

Extenuating Circumstances Tuition and Fees Refund Request

This form is utilized by students who are wanting to file an exception to the tuition and fees refund policy due to extenuating circumstances. This form must be filled out and submitted to our office with documentation of the extenuating circumstance.

Iowa Voter Registration Application

This form is utilized by an individual who wishes to register to vote in the State of Iowa. It is important to note that the individual utilizing this form must be a United States citizen and 18 years of age or older on or before the day of the election in which they intend to participate. For more information related to the use of this form, please refer to the Iowa Secretary of State.

Name Change Form

Students can utilize this form to change how their name is listed in the university's student information system. The name on the student record should be the student's complete and legal name. Once this form is filled out and signed, it should be submitted to our office for processing. For more information related to the University's name change policy, please refer to "What is the University's name change policy?" under the Students FAQ's.

National Mail Voter Registration Application

This form is utilized by an individual who wishes to register to vote. It is important to note that the individual utilizing this form must be a United States citizen and 18 years of age or older on or before the day of the election in which they intend to participate. For more information related to the use of this form, please refer to the U.S. Election Assistance Commission.

Post Baccalaureate Application for Admission (New to UNI Student)

The Post Baccalaureate Application for Admission is utilized by an individual with a Baccalaureate Degree from an institution other than the University of Northern Iowa who wishes to enter UNI as an undergraduate student. Prior to submitting the Post Baccalaureate Application for Admission, it is strongly recommended that you read "Second Bachelor's Degree" under the "Visitors" tab.

Post Baccalaureate Application for Admission (Previous UNI Student)

The Post Baccalaureate Application for Admission is utilized by a previous University of Northern Iowa student with a Baccalaureate Degree who wishes to enter UNI as an undergraduate student. Prior to submitting the Post Baccalaureate Application for Admission, it is strongly recommended that you read "Second Bachelor's Degree" under the "Alumni & Friends" tab.

Registration Form

This form is utilized by an individual who wishes to register for one or two classes by mail as opposed to electronically through their MyUNIverse account. Before we can process a registration request, this form must be filled out, signed, and returned to our office via fax (319-273-6792), mail (Office of the Registrar, 115 Gilchrist Hall, Cedar Falls, IA  50614-0006), or scan and e-mail (Registrar@uni.edu). Prior to submitting the Registration Form, it is strongly recommended that you read "Registration" under the "Students" tab.

Resident Classification Application

This form is utilized by a student who wishes to request a change in their residency classification. Before we can process the Resident Classification Application, this form must be filled out, signed, and returned to our office via fax (319-273-6792), mail (Office of the Registrar, 115 Gilchrist Hall, Cedar Falls, IA  50614-0006), or scan and e-mail (Registrar@uni.edu). Prior to submitting the Resident Classification Application, it is strongly recommended that you read "Residency Determination," "Guidelines for Determining Residency," and "General Information Regarding Residency" under the "Students" tab.

Transcript Request Form

The Transcript Request Form is utilized by an individual who wishes to order an official transcript from the University of Northern Iowa. Before we can process a request for an official transcript, the Transcript Request Form must be filled out, signed, and returned to our office. Prior to submitting the Transcript Request Form, it is strongly recommended that you read "Transcript Requests" under the "Students" tab.

Undergraduate Student Request

This form is utilized by an undergraduate student for a variety of requests, including, but not limited to: (1) Adding a course after a deadline published in the Schedule of Classes; (2) Dropping a course with a "W" after the deadline published in the Schedule of Classes; (3) Registering for an overload; and (4) Receiving graduate credit as a senior.

Withdrawal Form

This form is utilized by a student who wishes to withdrawal from all of their classes at the University of Northern Iowa.  Prior to submitting the Withdrawal Form, it is strongly recommended that you read "Withdrawal Schedule" under the "Calendars" tab. You should also read "Withdrawal: Refund Percentages and Grade Assignments" under the "Tuition & Fees" tab.
