Student of the Month Award Nomination Form

The Rod Library Student Employment Advisory Committee would like to name a "Student Assistant of the Month" as a way to recognize and reward outstanding student assistants in the Library. Faculty, staff, fellow student assistants and library patrons may complete this form to nominate student assistants who have displayed exemplary performance in areas of their employment. Please be as specific as possible as this information will, in a large part, form the basis for the advisory committee's choice for the award.
