Immunizations
Measles (Rubeola), Mumps and Rubella are preventable diseases. To eliminate the possibility of an outbreak, UNI does require all students show proof
of two MMR immunizations. Students who have applied for admission to UNI will receive the Immunization Requirement Form with their provisional acceptance letter from the Office of Admissions.
MMR is a combination vaccination for Measles (Rubeola), Mumps and Rubella.
Your immunization records can be obtained from your health care provider’s office. They may also be available from your high school or other colleges or universities you have attended. Public health records or military records may also have this information. Please note all the immunization dates and tests must include month, day and year. A photocopy of your immunization record is required and should be attached to the immunization form.
If you need a measles immunization the MMR is available at the SHC. Call (319) 273-2009 for an appointment. New students are eligible for vaccination and other services beginning the day of registration.
