APPLICATION MATERIALS

To apply for the Classic Upward Bound Program, students must prepare an Application Packet.  To download copies of application forms, click on the following links:

  Student's Recent Grades/Transcript

  Student Reference Form - Print out 2 copies, to be

     signed by teacher and/or counselor

  Permission to Access Records Form

  Confidential Release for Verification of

     Non-Taxable Income, or

     A copy of Parents' 1040, 1040A, or 1040EZ tax form
