Reader/Scribe Request Procedure (PDF)

Students should try to make requests for readers and/or scribes at least one week prior to a scheduled exam. Request forms can be picked up from Student Disability Services, printed from our website, or completed using the online Word form and emailed to our office.

 

REQUEST PROCEDURE

1. Complete a Reader/Scribe Request Form.

2. Bring your requests to Student Disability Services or complete the Word form, attach it to an email, and send it to disabilityservices@uni.edu.

3. SDS will contact you when an individual has been scheduled to read or scribe for your exam.
