The Dean of Students Office is committed to helping students succeed. In recognition of the unique needs and interests of LGBT students, this form is available to provide students a platform to submit questions and concerns without judgment or fear. Students may ask questions regarding the following:

• Who to talk to if you are having issues with classes
• Are in need of connection with someone for advice or support, etc
• Are in need of information regarding an incident of discrimination and/or harassment and are not sure of the policies, procedures, etc.

 Any immediate concerns, such as if you are a victim of sexual harassment, rape, or are in need of immediate emotional or physical assistance should contact the UNI Counseling Center at (319) 273-2676 or if after 5pm on weekdays or on weekends contact UNI Public Safety at (319)-273-2712.
