To request that your credentials be sent to an employer, please complete each section of the following form and click on the submit button. Your request will be forwarded to us by email. You will be assesed a $5.00 fee for each set that we send at your request. If you have any questions contact the UNI Career Services at (319) 273-6857 or at education.placement@uni.edu.
