Cooperative Education/Internship Faculty Approval Form

Instructions for students: Prior to entrance into the Co-op/Intern Program, you must obtain permission from the designated faculty co-op/intern supervisor in the academic department from which you seek credit. Check the "Departmental Requirements" for the correct faculty co-op/intern supervisor at www.uni.edu/careerservices/co-op/requirements/index.html

1. Complete this form and click to submit.

2. After you have clicked the submit button an e-mail will be sent to the UNI Career Services with your information. NOTE: You must have a minimum 2.5 GPA to participate in the Co-op/Intern Program. If you have a question regarding your GPA please check with the Co-op/Intern Office at 214 East Bartlett Hall.

3. After your information is processed and faculty approval has been received, you will receive an e-mail at the e-mail account provided by you below with further instructions on finalizing your approval.
